This website is operated by Optimum Health and Fitness Centre (Pty) Ltd, at 1 Power Road, Reitzpark, Welkom, Free State, South Africa. Contact info@optimumhfc.com or 065 554 7462. It provides information and a way to prepare booking, quotation and service enquiries.
Bookings and confirmation
A preferred date, a completed form or a prepared message is a request, not a reservation or confirmed appointment. Complete sending through WhatsApp or email. Our team confirms availability, service requirements, location and any quotation or payment arrangements separately. Walk-ins are welcome during clinic hours, subject to the clinical team’s ability to assist.
On-site assessments at a client’s workplace, after-hours visits and weekend appointments require advance booking. For workplace visits, the required space, privacy, equipment and site arrangements must be agreed with our team.
Quotations and payment
Services are quoted according to the required scope. Confirm the tests, employee numbers, location, any third-party investigations and payment terms before accepting a quotation. No payment is taken through this website. Any specific service agreement or accepted quotation must be read together with applicable law.
Changes and cancellations
Notify bookings@optimumhfc.com or call us as soon as you need to change a booking. Rescheduling depends on availability. Any lawful cancellation or other charges must have been disclosed and agreed for the relevant booking; these website terms do not create an automatic fee.
Accurate information and employer arrangements
Provide accurate contact, job and exposure information, and only details you are authorised to share. Employers arranging group medicals should nominate a contact, confirm employee requirements and coordinate the requested referral or exposure documents. Keep clinical records and identity numbers out of the website form.
Clinical judgement and fitness outcomes
Assessments are performed within the practitioner’s professional scope and according to the relevant job, exposure and clinical requirements. A booking or payment does not guarantee a fit outcome, a particular certificate or a fixed turnaround time. Further tests, referral or review may be necessary. The practitioner will explain the next steps.
Website information is general service information. It does not diagnose a condition, replace individual clinical advice or provide emergency care. Do not use the enquiry form for an urgent medical situation; contact appropriate emergency services.
Privacy and consent
The privacy policy and cookie policy explain the website’s information handling. Consent to contact you about an enquiry is separate from clinical consent, employer reporting authorisation and any optional marketing consent.
